Work Environment

The role is primarily remote, designed to offer flexibility while maintaining high standards of patient care. However, you will also collaborate with colleagues in our outpatient office, participating in team meetings and case discussions. Key Responsibilities

• Conduct thorough psychosocial assessments to identify patients’ needs, determining the most appropriate treatment plans and referral processes.
• Provide short-term counseling and crisis intervention to assist patients in acute distress, employing both telephone and video methods when necessary.
• Guide patients in successfully navigating forms related to entitlements and supplemental insurance, easing their transition from reliance on emergency assistance programs.
• Assist in obtaining prescription cost assistance through available indigent care programs and community resources, empowering patients to manage their healthcare costs effectively.
• Advocate for patients by liaising with various community agencies to facilitate access to needed services and supports.
• Maintain accurate and up-to-date patient records in accordance with regulatory standards, preparing detailed reports and documentation to support ongoing care strategies.
• Coordinate transportation assistance for patients who require it, ensuring they have access to scheduled appointments.
• Actively participate in clinical meetings related to case conferences, sharing insights and developing best practices in collaborative treatment planning.

Patient Population / Client Focus

You will be serving a diverse patient population, including individuals undergoing mental health challenges such as anxiety, depression, trauma, and substance use disorders. Your ability to demonstrate empathy and tailored therapeutic approaches will directly improve our patients’ coping strategies and overall mental health outcomes.

Qualifications

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.
• An active and independent license is required, with options including Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
• A minimum of two (2) years of professional experience in a relevant mental health setting is essential.

Preferred Experience

Experience in case management and working with marginalized populations is preferred. Familiarity with telehealth platforms and virtual counseling techniques will be an advantage as many patient interactions will occur in a remote setting.

Compensation and Benefits
