Key Responsibilities
  • Conduct comprehensive assessments of patients' psychosocial needs, creating tailored treatment plans that address individual challenges and optimize mental wellness.
  • Provide crisis intervention and short-term counseling as needed to ensure immediate support for patients in distress.
  • Assist patients in completing necessary documentation to access benefits such as entitlements, supplemental insurance, and resources that can reduce their reliance on emergency aid.
  • Facilitate access to prescription assistance programs through community resources and pharmaceutical initiatives, ensuring patients receive their medications without financial burden.
  • Advocate for patients by collaborating with community agencies, ensuring they have the resources and support necessary to navigate healthcare systems effectively.
  • Maintain clear, accurate, and up-to-date patient records in compliance with regulations and organizational protocols, preparing reports as required.
  • Coordinate transportation assistance for patients when necessary to improve access to care, reflecting our commitment to patient-centered services.
  • Participate in clinical team meetings to discuss case progress and strategies, promoting a collaborative approach to treatment.
Patient Population / Client Focus

This role primarily serves a diverse range of patients in the Sacramento area, including those facing severe mental health challenges, economic hardships, and complex psychosocial barriers. Your ability to understand and respond to various cultural backgrounds and mental health needs will be vital in fostering trust and rapport with clients.

Qualifications
  • A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.
  • Possession of an active and independent license, such as Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
  • A minimum of two (2) years of professional experience in mental health counseling or a related field is essential.
Preferred Experience

Ideal candidates will have experience in case management, crisis intervention, and community resource coordination. Familiarity with telehealth platforms and the ability to engage effectively with clients using digital communication tools are also highly regarded.

Compensation and Benefits
