Key Responsibilities

• Perform comprehensive assessments of patients' psychosocial needs and develop personalized treatment plans that may include short-term counseling and crisis intervention.
• Assist patients in navigating complex processes to obtain necessary entitlements, supplemental insurance, and access to various benefits, thus promoting self-sufficiency.
• Help patients gain access to prescription cost assistance programs offered by pharmaceutical companies and community resources, ensuring they receive the medications they require.
• Actively advocate for patients during interactions with community agencies, fostering relationships that facilitate the provision of essential services.
• Maintain accurate and up-to-date patient records, preparing detailed reports and documentation to support clinical and administrative requirements.
• Coordinate necessary transportation assistance for patients, minimizing barriers to accessing care.
• Participate in regular clinical and team meetings, collaborating with your peers on case conferences and sharing insights to enhance patient care.

Patient Population / Client Focus

This role primarily serves adults facing a range of mental health issues, including anxiety, depression, trauma, and substance use disorders. As a Licensed Mental Health Therapist, you will work with diverse clientele from various backgrounds, empowering them to develop coping strategies and improve their overall mental well-being. Your efforts will directly contribute to improving patient outcomes and enhancing resilience across our community.

Qualifications

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.
• Possession of one of the following active, independent licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
• Minimum of two (2) years of professional experience in a mental health counseling role, demonstrating a strong understanding of therapeutic interventions.

Preferred Experience

• Prior experience in outpatient settings, especially working with diverse populations.
• Familiarity with case management systems and electronic health record (EHR) platforms.
• Experience with telehealth services is highly desirable, particularly in engaging clients in a remote setting.

Compensation and Benefits
