Work Environment

This position is entirely remote, enabling you to work from the comfort of your home while maintaining a healthy work-life balance. We provide all necessary tools and resources to facilitate effective online communication and collaboration among team members. Our hours of operation are Monday to Friday, 9:00 AM to 5:00 PM, aligning with a structured full-time schedule that supports consistent engagement with clients and families.

Key Responsibilities
  • Serve as a liaison between clientsโ€™ families and outpatient providers to ensure cohesive care and support.
  • Engage with insurance companies for pre-certifications and utilization management, advocating for necessary services on behalf of clients.
  • Participate in discharge and aftercare planning, creating individualized support plans that promote successful transitions.
  • Offer therapeutic meal support, addressing food-related issues while modeling a healthy relationship with food, vital for clientsโ€™ overall well-being.
  • Deliver safe, empathetic, and affirming care tailored to each clientโ€™s unique circumstances and intersecting identities, significantly enhancing their experience within our hospice care setting.
Patient Population / Client Focus

In this role, you will primarily work with older adults facing terminal illnesses, along with their families. Your responsibilities will include addressing their emotional, social, and practical needs, ensuring that both clients and their loved ones feel supported and understood during this critical time. You will utilize your skills to help clients navigate complex emotions, cope with grief, and access community resources that enhance their quality of life.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Must possess a valid LCSW license in the state of Texas.
  • Experience working with older adults in healthcare or hospice settings is essential.
  • Strong clinical and diagnostic skills, with the ability to assess and create treatment plans effectively.
  • Exceptional communication and interpersonal abilities to foster strong relationships with clients, families, and other healthcare professionals.
  • Ability to work independently, demonstrating strong initiative and contribute positively as a collaborative team member.
Preferred Experience
  • Experience in hospice care or with palliative services provides a valuable perspective on the needs of clients in these settings.

Compensation and Benefits

The salary range for this position is competitive, offering between $90,000 to $105,000 annually, commensurate with experience and qualifications. Our benefits package includes:
