In this remote role, you will primarily engage with clients through telehealth platforms, allowing for flexibility while maintaining a high standard of care. The Hospice Center environment is one of empathy and understanding, where you will work collaboratively with a team of healthcare providers, including nurses, physicians, and palliative care specialists. • Conduct comprehensive psychosocial assessments to evaluate the mental health needs of clients, ensuring that all factors impacting their well-being are considered.
• Develop and implement customized care plans that align with clients' goals, utilizing evidence-based therapies and interventions tailored to their specific needs.
• Facilitate individual and group therapy sessions, employing therapeutic techniques that foster trust and engagement while addressing clients' emotional, social, and spiritual concerns.
• Work collaboratively with an interdisciplinary team to provide integrated care, participate in regular case discussions, and contribute to treatment planning.
• Advocate for clients' access to community resources, ensuring they receive support for practical needs associated with their healthcare journey.
• Maintain detailed records of client interactions and treatment outcomes in electronic health record systems, ensuring compliance with confidentiality and regulatory standards.
• Continuously seek professional development opportunities through training sessions and workshops to refine your skills and stay updated with the latest practices in social work.

Your primary focus will be on clients in the hospice setting, often including older adults and their families. You will provide comfort and support during difficult transitions, addressing complex emotional challenges such as grief, loss, and adjustment to chronic illness. Your ability to develop rapport and provide compassionate care will be crucial in fostering a sense of security among patients and their families.

• A Master’s degree in Social Work from an accredited institution is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• At least 2 years of clinical experience, specifically in working with older adults or in a hospice/palliative care setting.
• Demonstrated strong clinical assessment and diagnostic skills.
• Excellent communication abilities and interpersonal skills to establish effective relationships with clients and team members.
• A demonstrated capacity to work independently while contributing to a collaborative team environment.

• Experience in grief counseling or bereavement support, ideally within a hospice or healthcare environment.
• Familiarity with electronic health record systems (EHR) and telehealth platforms.
• Active involvement in professional social work organizations.

We offer a competitive salary range of $110,000 to $130,000 annually, commensurate with experience and qualifications. Our benefits package includes:
