This full-time remote role focuses on providing clinical support within a detox center, ensuring that our patients receive comprehensive care tailored to their unique bio-psychosocial needs.

Key Responsibilities
  • Conduct thorough assessments of patients to identify psychosocial issues and develop tailored treatment plans.
  • Provide individual and group counseling sessions, utilizing evidence-based practices to facilitate recovery.
  • Assist patients in navigating the complexities of accessing health benefits, including completion of entitlement forms and navigating insurance options.
  • Advocate for patients by collaborating with community agencies to secure necessary support and resources.
  • Maintain meticulous patient records and documentation in compliance with healthcare regulations and standards, ensuring accurate reporting and follow-up.
  • Coordinate patient transportation and other logistical support to enhance accessibility to care.
  • Engage in case conferences and team meetings, contributing insights and strategies to improve patient outcomes.
  • Facilitate patient education programs, particularly focusing on prenatal and postpartum health, as well as HIV counseling.
  • Conduct regular follow-ups with patients to monitor progress and address concerns such as missed appointments.
