• Engage directly with clients diagnosed with major mental illnesses or severe emotional disorders, both in office settings and the community.
• Develop and implement individualized care plans tailored to each client's unique needs, ensuring access to appropriate Linked services.
• Conduct evidence-based screenings and assessments, identifying client needs to recommend further services or intervention as required.
• Deliver brief, focused prevention, treatment, and recovery services, adaptable to the diverse needs of clients.
• Facilitate individual, group, and family therapy sessions, emphasizing the importance of choice and empowerment in the therapeutic process.
• Assist with crisis interventions, utilizing best practices to ensure client safety and well-being during critical moments.
• Manage a defined caseload, tracking treatment progress, monitoring criteria for admission and discharge, and ensuring timely follow-up.
• Collaborate with interdisciplinary teams to enhance service delivery and improve client outcomes.
• Maintain diligent records and documentation, adhering to ethical guidelines and organizational standards.
• Provide on-call support as needed outside of regular program hours, demonstrating your commitment to client care.

Your primary focus will be working with individuals facing complex mental health issues in under-served communities, ensuring that every patient receives culturally competent care. Understanding the varying traditions, beliefs, and interpersonal styles of your clients will be crucial in fostering trust and effective therapeutic relationships.

• A Bachelor’s Degree in Social Work (BSW) is required; a Master’s Degree in Social Work (MSW) is preferred.
• Licensed as a Clinical Social Worker (LCSW) in Colorado.
• A minimum of two years of relevant experience in mental health settings.
• Demonstrated ability to establish rapport with diverse clients and team members, emphasizing trust and mutual respect.
• Proficient in evidence-based practices, mental screening tools, and integrated care methodologies.
• Knowledge of crisis intervention techniques and safety protocols is essential.

• Experience working in a hospice or end-of-life care setting is highly desirable.
• Familiarity with community resources and services available to support client needs.
• Prior engagement in multidisciplinary teams to improve service delivery and patient outcomes.

The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
