• Develop and coordinate individualized treatment plans in collaboration with healthcare professionals, drawing on comprehensive psychosocial assessments to inform patient discharge processes.
• Conduct thorough assessments of patients’ needs and implement appropriate interventions, while maintaining accurate and timely clinical records in accordance with department policies and electronic medical records systems.
• Provide crisis intervention and therapeutic counseling for patients facing emotional and behavioral challenges, ensuring that care integrates both therapeutic modalities and practical solutions.
