This full-time, remote position within our Hospice Center offers the opportunity to provide vital therapeutic services and case management to individuals and families facing mental health challenges. This role will primarily be conducted in a remote setting, allowing you to connect with clients and the team from the comfort of your home. Flexibility in scheduling can accommodate work-life balance, yet you will collaborate closely with colleagues and access resources to assist clients effectively. While you will utilize telehealth technologies for sessions, in-house training and professional development opportunities will ensure that you remain at the forefront of best practices in social work and mental health support.

• Provide individual, group, and family therapy sessions utilizing evidence-based practices tailored to each client’s unique situation and needs.
• Conduct comprehensive psychosocial assessments that inform the development of personalized treatment plans.
• Deliver crisis intervention support, offering immediate responses and coping strategies for clients in acute distress.
• Guide clients in accessing vital community resources, including housing support, healthcare services, and financial assistance programs.
• Collaborate with interdisciplinary teams, including psychiatrists, therapists, and medical professionals, ensuring coordinated and holistic client care.
• Maintain precise and thorough clinical documentation compliant with HIPAA standards, contributing to overall quality service provision.
• Advocate for client rights, emphasizing emotional well-being and long-term mental health goals within community contexts.

You will work with diverse individuals and families facing a range of mental health challenges, particularly those navigating the complexities associated with hospice and end-of-life issues. Your role will be pivotal in providing empathy and support, helping these clients find peace and coping mechanisms during distressing times. Your ability to make connections and understand unique circumstances will directly impact client outcomes and their families’ experiences.

• Master's degree in Social Work (MSW) from an accredited educational institution.
• Active LCSW license in Georgia, with a strong ethical framework.
• A minimum of 2 years of clinical experience in mental health or in related social services.
• Proficient in case management as well as therapeutic interventions, demonstrating crisis de-escalation techniques effectively.
• Familiarity with trauma-informed care principles, motivational interviewing strategies, and cognitive-behavioral therapy (CBT) methods.
• Exceptional communication and problem-solving skills, adaptable to diverse client needs.
• Ability to work autonomously while also contributing positively to a collaborative multidisciplinary team environment.

• Experience working in hospice or palliative care settings.
• Knowledge in working with bereavement and loss counseling.
• Familiarity with Electronic Health Record (EHR) systems and telehealth platforms.
