Position Overview

Alvita Counseling Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our esteemed hospice team on a full-time basis. Work Environment

Our work setting is predominantly remote, allowing our social workers to provide care from the comfort of their own homes while remaining actively involved in the lives of our patients and their families. Despite the remote nature of this role, you will be part of a closely-knit interdisciplinary team of healthcare professionals, including nurses, physicians, and other specialists who collaborate to create a holistic care environment focused on patient-centered outcomes.

Key Responsibilities
  • Conduct in-depth psychosocial assessments through patient and family interviews, observations, and a review of medical histories to identify emotional, social, and environmental factors affecting the recovery and comfort of patients.
  • Deliver counseling services and provide support to patients and their families, assisting them in coping with end-of-life issues, adjustment to terminal diagnoses, and grief.
