• Collaborate closely with a multidisciplinary team, including medical professionals and counselors, to design and implement individualized care plans that meet the holistic needs of each patient.
• Conduct comprehensive assessments to evaluate the psychological, social, and emotional aspects of patients’ conditions, delivering tailored interventions and strategies.
• Provide individual and family counseling, helping patients and their loved ones cope with the emotional aspects of terminal illness and end-of-life considerations.
• Educate patients and their families about available resources, treatment options, and coping mechanisms, ensuring they are empowered and informed throughout their journey.
• Stay current with the latest research and therapeutic practices in mental health to enhance service delivery and patient outcomes.

You will be working primarily with patients receiving hospice care, focusing on individuals facing end-of-life decisions and their families. This patient population requires a profound understanding of grief, loss, and the complex emotions that accompany terminal illness. You will be instrumental in providing support, facilitating communication among family members, and ensuring that their voices are heard throughout the care process.

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• Active Licensed Clinical Social Worker (LCSW) license in Virginia.
• Robust clinical experience, ideally with a minimum of 2 years in palliative care or hospice settings, is preferred.
• Demonstrated expertise in clinical evaluations, treatment planning, and a deep understanding of mental health issues related to terminal illnesses.
• Strong communication, interpersonal, and teamwork skills are essential, with the ability to establish rapport and trust with patients, families, and colleagues alike.

• Experience working with diverse populations in high-stress or crisis situations.
• Familiarity with electronic health record (EHR) systems to ensure accurate patient documentation and data management.
• Knowledge of local and state resources available for end-of-life care and mental health support.

The annual salary for this position ranges from $90,000 to $110,000, commensurate with experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
