• Conduct thorough psychosocial assessments to evaluate patients' mental health status, identifying their strengths, challenges, and specific needs.
• Deliver evidence-based therapeutic interventions, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), tailored to individual preferences and conditions.
• Collaborate with a multidisciplinary team composed of psychiatrists, nurses, and allied health professionals to create and modify comprehensive care plans.
• Provide educational support to patients and families about mental health diagnoses, treatment options, and potential side effects of medications to facilitate informed decision-making.
• Facilitate individual, group, and family therapy sessions that foster emotional healing and provide coping strategies.
• Develop treatment plans aligned with patients' goals, using collaborative approaches to ensure treatment efficacy.
• Act as an advocate for patients, assisting them in navigating the complexities of mental health services and community resources.
• Lead therapeutic groups and educational workshops that empower patients and families during the hospice journey, addressing both emotional and practical concerns.

You will be primarily working with patients who are in hospice care, experiencing advanced illnesses. This vulnerable population often requires specialized attention to mental health issues related to grief, anxiety, and depression. Your expertise will be crucial in helping them cope with their circumstances and improve their overall quality of life during this challenging time.

• A Master's degree in Social Work (MSW) from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of mental health disorders, treatment methodologies, and crisis intervention strategies.
• Exceptional communication skills and the ability to work effectively within a multidisciplinary team.

• Prior experience in hospice, palliative care, or similar settings dealing with end-of-life issues.
• Familiarity with telehealth technologies and virtual therapy modalities.
• Experience in group therapy facilitation and psychoeducation.
