Your role as a Licensed Professional Counselor will involve a wide range of responsibilities aimed at addressing mental health and psychosocial issues effectively:

• Collaborate with primary care practitioners to identify, treat, and manage mental health conditions, contributing to an all-encompassing care model that optimizes patient health.
• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, to formulate individualized therapy objectives and recommend treatments to primary care colleagues.
• Provide timely, crisis-oriented mental health interventions for patients in need of emergency support, ensuring their safety and stability.
• Guide primary care providers in determining which patients require focused mental health services and facilitate appropriate referrals to specialists when necessary.
• Regularly evaluate patient progress, adapting treatment plans based on efficacy and required modifications, to sustain optimal therapeutic outcomes.
• Work with patients to develop relapse prevention strategies and support mechanisms that assist them in their journey to wellness.
• Identify patients at risk for declining health and actively participate in preventative program initiatives tailored to their specific needs.
• Oversee the coordination of mental health services, ensuring seamless communication with both primary care and external treatment providers when appropriate.
• Maintain precise documentation of patient assessments and progress in treatment charts, adhering to compliance and confidentiality standards.
• Engage actively with primary care colleagues to share updates on patient status and collaborate on treatment planning.
• Empower patients by providing essential self-management skills and educational resources that enable their active participation in care.
• Consult with clinic management and team members to identify opportunities for program enhancement and outcome evaluation.
• Support patients in adhering to prescribed treatment plans from their primary care providers, including management strategies for medication side effects.
• Uphold the Clinical Professionals' Code of Ethics in all interactions and responsibilities while being flexible to perform additional duties as assigned.

To be successful in this role, candidates must possess the following qualifications:

• Proficiency in both English and Spanish is desirable but not mandatory.

We offer a competitive salary range of USD 90,000 to 105,000 per year, commensurate with experience and education.
