This position allows you to work remotely while providing your services at a hospice center. Key Responsibilities
  • Conduct comprehensive assessments to determine the mental health and psychosocial needs of clients, documenting observed behaviors, emotional states, and situational factors that influence their well-being.
  • Provide individualized, one-on-one counseling sessions, employing therapeutic modalities tailored to the specific needs of clients, including grief counseling, crisis intervention, and emotional support.
  • Organize and facilitate group therapy sessions that target common themes such as grief processing, coping strategies, and peer support, creating a safe space for sharing and healing.
  • Develop and oversee personalized treatment plans, setting achievable, measurable goals, and utilizing evidence-based interventions to track progress effectively.
  • Offer crisis intervention services, including conducting risk assessments, creating safety plans, and coordinating access to emergency resources and mental health services when necessary.
  • Advocate for clients’ rights, helping them navigate complex healthcare systems and ensuring they have access to supportive resources in their community.
  • Maintain meticulous and confidential client records, adhering to all legal and ethical standards in record-keeping practices, including treatment notes and outcome evaluations.
Patient Population / Client Focus

Your work will primarily focus on patients in a hospice care setting who are managing terminal illnesses. This unique client population presents distinct emotional and psychosocial challenges, requiring a sensitive approach to end-of-life issues, grief, and loss. You will play a critical role in helping patients and their families find emotional support and coping strategies while navigating this difficult period.

Qualifications
  • Master's degree in Social Work (MSW) from an accredited institution is mandatory.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
  • Prior clinical experience in a mental health or social work capacity, particularly within hospice or palliative care, is preferred.
