This position operates in a remote setting, allowing for flexibility while working closely with the hospice center's interdisciplinary team. • Conduct thorough psychosocial assessments of hospice clients, considering their emotional, mental, and social needs, and develop individualized care plans.
• Offer counseling and emotional support to both clients and their families, helping them navigate the complexities of end-of-life issues.
• Advocate for clients’ rights and access to community resources, ensuring a well-rounded support system is in place.
• Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate care, monitor client progress, and address any emerging concerns.
• Facilitate family meetings to improve communication, resolve conflicts, and strengthen family support structures during this challenging time.
• Educate clients and their families about available resources, support groups, and programs to enhance their emotional and social engagement.
• Maintain comprehensive records of client assessments, care plans, and outcomes, ensuring compliance with regulatory requirements and ethical standards.
• Stay informed about best practices in hospice care, including legislative changes impacting social work and end-of-life planning.

Your primary focus will be on seniors receiving hospice care, many of whom may be experiencing significant emotional distress due to illness, transition, or the grieving process. You will work alongside families who are navigating these challenges, providing essential support that enhances their overall well-being.

• Master's degree in Social Work from an accredited university.
• Current and valid LCSW licensure in the state of Arizona.
• A minimum of two years experience in social work, specifically in hospice, palliative, or geriatric care settings.
• Demonstrated ability to empathize and communicate effectively with clients in crisis situations.
• Thorough understanding of social work principles and practices, particularly as they pertain to aging-related challenges.
• Effective problem-solving, assessment, and counseling skills.
• Proficiency in electronic health record systems and ability to work with various software applications.

• Experience in hospice or end-of-life care settings.
• Knowledge of community resources available to seniors and their families.
• Experience working in an interdisciplinary team, collaborating with healthcare professionals to design and implement care plans.
• Previous involvement with grief counseling or psychoeducation.
