This full-time position offers the flexibility of remote work while playing a pivotal role in supporting patients and their families in a hospice setting. Our ideal candidate is motivated by the desire to enhance the quality of life for individuals dealing with complex emotional and psychosocial challenges.

About the Organization

Melodic Mind Diagnostic and Integrated Solutions is a pioneering healthcare provider based in Lake Oswego, Oregon, committed to delivering comprehensive mental health and addiction services. We strive to create an environment where patients feel valued, respected, and understood. Our approach integrates evidence-based practices with empathetic care, making a significant impact on community health.

Work Environment

This position is fully remote, allowing you the flexibility to balance personal and professional commitments while delivering care from the comfort of your home. Despite the remote nature of the role, you will be an integrated member of our dynamic care team through regular virtual meetings, case discussions, and collaborative planning sessions.

Key Responsibilities

As a Licensed Clinical Social Worker with Melodic Mind, you will:

  • Conduct thorough intake and psychosocial assessments to evaluate patients' needs and circumstances, ensuring comprehensive understanding from the outset.
  • Develop, present, and refine personalized treatment plans in collaboration with the Chemical Dependency and Recovery Program (CDRP) and treatment team members, ensuring alignment with best practices in addiction recovery.
  • Engage actively in clinical case conferences, contributing your insights and facilitating interdisciplinary collaboration for optimal patient care.
  • Deliver educational lectures focused on chemical dependency, enhancing patientsโ€™ understanding of their conditions and empowering them to engage in their recovery process.
  • Provide individual and group counseling services that are centered on chemical dependency, promoting healing and recovery in a supportive environment.
  • Facilitate therapy sessions for adolescents, significant others, and families, using tailored therapeutic approaches to address diverse needs.
  • Manage cases effectively by coordinating resources and referrals, ensuring patients have access to the services they require.
Patient Population / Client Focus

Your work will primarily involve patients facing terminal illnesses and their families, who are navigating emotional distress and complex psychosocial challenges. You will support these clients during a critical phase of their lives, helping them to achieve greater awareness and acceptance while fostering resilience through supportive counseling.

Qualifications

The successful candidate will possess:

  • A master's degree in social work (MSW) or a closely related field.
  • Current licensure as a Clinical Social Worker (LCSW) in the state of Oregon.
  • A minimum of two years of relevant supervised experience in addiction medicine therapy within the last five years, showcasing your expertise in handling complex cases.
Preferred Experience

While the minimum requirements are essential, we are particularly interested in candidates who have experience working within hospice settings or with other vulnerable populations. Familiarity with conducting group therapies and substance abuse education will be highly advantageous.

Compensation and Benefits

This position offers a competitive salary ranging from USD 115,000 to 130,000 annually, commensurate with experience and qualifications. In addition, our comprehensive benefits package includes:

  • Health, Dental, and Vision Insurance.
  • 401(k) plan with company match to support your long-term financial goals.
