Key Responsibilities

As a Licensed Professional Counselor at Oaks Integrated Health Centre, your primary responsibilities will include:

  • Conducting thorough and ongoing assessments to effectively meet the individual needs of patients, adapting approaches as necessary based on their feedback and progress.
  • Facilitating communication between all stakeholders involved in a client's care, including supervisors, direct care staff, families, and other relevant community members, to foster a comprehensive understanding of the patient's journey.
  • Creating and maintaining a tentative schedule for appointments, home visits, and observations to ensure that all aspects of care are monitored and documented properly.
  • Coordinating and conducting home visits, when necessary, to gather key observational data and insights that inform the therapeutic process.
  • Ensuring implementation of individualized program plans is conducted effectively, with regular feedback loops to uphold standard practices and therapeutic efficacy.
  • Participating in interdisciplinary team meetings to contribute perspectives and insights relevant to patient care and to collaborate effectively with healthcare professionals.
  • Completing detailed monthly and quarterly progress notes to track patient development and inform future therapeutic strategies.
  • Organizing training sessions aimed at equipping staff and other team members with the skills needed to implement program plans and document care accurately.
  • Offering consistent follow-up interviews and home visits to support ongoing patient needs, fostering long-term relational aspects of care.
  • Maintaining accurate and detailed records to ensure compliance with healthcare regulations and to facilitate collaborative care amongst the team.
  • Acting as a liaison within the public and mental health community to support program development and advocate for client needs.
