This remote position offers flexibility, allowing the LCSW to provide care in a virtual setting. Our hospice center prioritizes a supportive work culture, emphasizing collaboration with colleagues across various health disciplines. Work hours are structured to foster work-life balance, with a schedule from 9 AM to 6 PM, Monday to Friday, with no evening or weekend obligations, ensuring ample time for personal pursuits and rest.

• Conduct initial assessments to evaluate mental health needs and develop individualized treatment plans based on patient circumstances and goals.
• Provide ongoing psychotherapy to clients, utilizing evidence-based practices to address a range of mental health issues.
• Participate actively in a multidisciplinary care team, communicating effectively with primary care providers and other health professionals to coordinate comprehensive care.
• Implement and maintain compliance with mental health policies and procedures, ensuring high-quality service delivery within established standards.
• Facilitate crisis intervention and develop strategies for clients in distress, applying mature judgment to navigate urgent scenarios.
• Evaluate clients for potential referrals to inpatient or specialty care services, ensuring continuity of care and appropriate resource allocation.
• Conduct screenings for substance abuse, including illicit drugs and alcohol, and provide appropriate referrals to resources as needed.
• Assist clients who screen positive for mental health concerns by connecting them with localized support services.

The LCSW at Vitalize Metal and Wellness Center will work primarily with hospice clients facing a variety of mental health challenges, including but not limited to grief and loss, anxiety, depression, and life transitions. This role requires a deep understanding of the unique emotional and psychological needs of this population, fostering a therapeutic rapport and creating a safe space for healing.

• A Master’s degree in Social Work or Counseling from an accredited institution.
• Current LCSW licensure to practice in Connecticut.
• Proven clinical skills with a strong foundational knowledge of therapeutic techniques.
• Demonstrated maturity and professional judgment, particularly in crisis intervention.
• Excellent verbal and written communication skills with the ability to present information clearly.
• Strong team collaboration abilities to work effectively in a multidisciplinary environment.
• Proficiency in electronic health record (EHR) systems for maintaining accurate patient documentation.
• Experience working with culturally diverse populations, showing respect and sensitivity to various backgrounds.

• Previous experience in a hospice or palliative care setting is highly desirable.
• Familiarity with community resources and referral networks for mental health and substance abuse treatment.
• Experience implementing telehealth services or virtual therapy sessions.
