Position Overview

Garden Path Counseling is excited to announce an opening for a full-time Licensed Clinical Social Worker (LCSW) who will play a critical role in providing empathetic and professional mental health care to patients at our remote hospice center. The successful candidate will bring their expertise in clinical social work to deliver support to individuals navigating the complexities of mental health challenges, particularly within the unique setting of hospice care.

Work Environment

Key Responsibilities
  • Conduct thorough assessments and develop individualized treatment plans based on patients’ needs and circumstances, ensuring that care is tailored to each unique situation.
  • Facilitate engaging individual and group therapy sessions, employing various therapeutic techniques to help patients and their families cope with grief, anxiety, and related mental health issues.
  • Maintain meticulous documentation of patient progress and treatment interventions via our electronic health record system, adhering to all regulatory and organizational standards.
  • Actively collaborate with other healthcare professionals, including physicians and nurses, to coordinate care effectively and ensure holistic support for patients.
  • Provide crisis intervention and acute care management during emergent situations, ensuring responsiveness to the immediate mental health needs of patients and families.
  • Participate in regular case management meetings, reviewing client cases to enhance treatment planning and provide comprehensive care across various service levels.
Patient Population / Client Focus

You will work closely with individuals and families facing terminal illnesses, offering much-needed emotional and psychological support. The patient population includes adults of different age groups, who may present with a range of mental health concerns exacerbated by their medical situations. Your role will be pivotal in helping these clients navigate emotional distress, grief, and existential issues related to their life experiences.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution.
  • An active and current LCSW license from the state of Pennsylvania is required.
  • A minimum of 2 years of experience providing clinical social work services, particularly within mental health or hospice settings.
  • Proficiency in medical documentation practices, including familiarity with electronic health records.
Preferred Experience
  • Experience conducting outpatient therapy within a clinical setting, particularly with patients undergoing end-of-life issues.
  • Certified training in motivational interviewing and crisis intervention.
  • Expertise in working with children, adolescents, or addressing gender-specific counseling needs is valued.
