• Conduct in-depth clinical assessments, utilizing evidence-based practices to formulate individualized treatment plans that align with patients’ emotional and psychological needs.

• Deliver therapeutic interventions, including counseling and support, tailored to the palliative care requirements of patients and families managing complex emotions around grief and loss.

• Maintain meticulous clinical documentation, ensuring compliance with both state regulations and organizational policies, whilst fostering a transparent therapeutic relationship with clients.

• Collaborate actively with a multidisciplinary team to create coordinated care strategies, ensuring holistic support that addresses all dimensions of patient wellness.

• Participate in regular case consultations and peer review meetings, fostering an environment of continuous improvement and quality enhancement in care delivery.

• Educate patients and their families about coping strategies, mental health resources, and available support systems, thereby empowering them to navigate their unique care journeys.

• Stay abreast of emerging research, therapeutic techniques, and industry best practices to consistently enhance treatment outcomes for patients.

Your primary focus will be on patients receiving hospice care, many of whom may experience complex emotional challenges related to terminal illness. This role necessitates a strong ability to empathize with diverse backgrounds and individual situations, fostering a trusting environment where patients and families feel safe to share their feelings and concerns.

• Must possess a Master’s degree in Social Work from an accredited institution.

• Holds a valid LCSW-C license in the state of Maryland.

• A minimum of 2 years of clinical experience in a related field, demonstrating proficiency in psychotherapy and emotional support techniques.

• Exceptional communication skills, showcasing the ability to engage effectively with clients, families, and multidisciplinary teams.

While the minimum requirement is 2 years, applicants with additional experience in hospice or palliative care settings, as well as familiarity with solution-focused therapy, will be considered favorably.
