• Conduct thorough assessments to identify patients' needs, strengths, and challenges, which form the basis for developing personalized treatment plans.
• Implement evidence-based interventions, including solution-focused therapy and stress management techniques, to enhance the emotional and psychological well-being of patients.
• Maintain meticulous clinical records, ensuring compliance with confidentiality standards while accurately documenting treatment plans and patient progress.
• Collaborate with an interdisciplinary team that includes physicians, nurses, and other healthcare professionals to deliver holistic and effective patient care.
• Educate and empower patients and their families about available resources, coping strategies, and support systems throughout the hospice journey.

Your role will primarily focus on supporting patients who are in end-of-life care, ensuring they and their loved ones navigate this challenging time with dignity and peace. You will work with individuals experiencing a range of emotions, including grief, anxiety, and depression, necessitating a sensitive and individualized approach.

• A Master's Degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker - Clinical (LCSW-C) license in the state of Maryland.
• A minimum of two years of clinical experience, preferably in hospice or palliative care settings.
• Proficiency in therapeutic modalities, particularly solution-focused therapy, with a strong ability to adapt techniques to meet diverse patient needs.
• Exceptional communication and interpersonal skills, enabling effective interactions with patients, families, and multidisciplinary teams.
• Strong analytical and critical thinking capabilities, ensuring effective problem-solving in complex clinical situations.

• Prior experience working in a hospice care environment, particularly with terminally ill patients and their families.
• Background in trauma-informed care and psychosocial interventions.
• Experience with electronic health record (EHR) systems to manage clinical documentation efficiently.
