Key Responsibilities

  • Maintain meticulous clinical records, ensuring compliance with confidentiality standards while accurately documenting treatment plans and patient progress.

Patient Population / Client Focus

Your role will primarily focus on supporting patients who are in end-of-life care, ensuring they and their loved ones navigate this challenging time with dignity and peace. You will work with individuals experiencing a range of emotions, including grief, anxiety, and depression, necessitating a sensitive and individualized approach.

Qualifications
  • A Master's Degree in Social Work from an accredited institution.
  • A valid Licensed Clinical Social Worker - Clinical (LCSW-C) license in the state of Maryland.
  • A minimum of two years of clinical experience, preferably in hospice or palliative care settings.
  • Proficiency in therapeutic modalities, particularly solution-focused therapy, with a strong ability to adapt techniques to meet diverse patient needs.
  • Exceptional communication and interpersonal skills, enabling effective interactions with patients, families, and multidisciplinary teams.
  • Strong analytical and critical thinking capabilities, ensuring effective problem-solving in complex clinical situations.
Preferred Experience
  • Prior experience working in a hospice care environment, particularly with terminally ill patients and their families.
  • Background in trauma-informed care and psychosocial interventions.
  • Experience with electronic health record (EHR) systems to manage clinical documentation efficiently.
