Key Responsibilities
  • Conduct comprehensive clinical assessments to understand the psychosocial needs of patients and families, forming the basis for individualized treatment plans.
  • Develop and implement evidence-based therapeutic interventions tailored to each patient's needs, ensuring they are involved in goal-setting and care decisions.
  • Maintain efficient and accurate clinical documentation, tracking progress and ensuring compliance with state and federal regulations.
  • Collaborate closely with physicians, nurses, and other members of the care team to provide coordinated and holistic patient care.
  • Lead case consultations and participate in regular peer review meetings to assess care quality and share best practices.
  • Educate patients and their families about mental health resources, coping strategies, and the importance of support systems during this sensitive time.
  • Stay informed about the latest research and advancements in social work and palliative care to continually enhance service delivery and therapeutic approaches.
Patient Population / Client Focus

Your clientele will consist mainly of terminally ill patients and their families who face emotional, social, and existential challenges while receiving hospice care. You will be instrumental in addressing the complex psychological impacts of end-of-life issues, providing empathy and professional support that encourages a peaceful transition and enhances the quality of life for both patients and families.

Qualifications
  • A Master’s Degree in Social Work (MSW) from an accredited institution.
  • A valid LCSW-C license in the state of Maryland.
  • At least 2 years of clinical experience, particularly in hospice or palliative care settings.
  • Strong competency in solution-based psychotherapy interventions and a commitment to ethical practice standards.
  • Exceptional communication skills, both verbal and written, with the ability to foster trust and rapport with clients.
  • A collaborative mindset and the ability to thrive in a multidisciplinary team setting.
Preferred Experience
  • Three or more years in clinical practice, especially focusing on psychotherapy and crisis intervention in grief care.
  • Experience working with diverse cultural backgrounds and a deep understanding of cultural competencies in delivery of care.
  • Familiarity with hospice and end-of-life care philosophies.
