Position Overview

Healthy Life Reformation & Counseling Center is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team in a fully remote capacity. This full-time role involves engaging with clients in a hospice care setting, ensuring they receive the highest quality of mental health support. You will play a crucial role in addressing and managing the psychological needs of individuals facing life-limiting illnesses, with a holistic approach to promote their emotional and mental well-being.

As part of our hospice care team, you will work collaboratively with healthcare professionals including physicians, nurses, and other therapists to provide comprehensive support to clients and their families. Key Responsibilities
  • Conduct thorough psychosocial assessments to understand the unique needs of each client facing end-of-life issues.
  • Offer individual, family, and group therapy sessions through telehealth services, employing evidence-based treatment modalities such as Cognitive Behavioral Therapy, Solution-Focused Therapy, and Trauma-Informed Care.
  • Develop and implement individualized treatment plans that outline specific goals, interventions, and expected outcomes, adjusting plans as necessary to meet evolving client needs.
  • Collaborate closely with physicians, nurses, and other mental health professionals to ensure a coordinated approach to holistic client care.
  • Provide crisis intervention strategies, risk assessments, and safety planning for clients in distress.
  • Maintain meticulous documentation in compliance with ethical and legal standards, ensuring all records are up-to-date and reflective of client progress.
  • Facilitate group therapy sessions and educational workshops that empower clients and their families by providing psychoeducation on grief, coping strategies, and other relevant topics.
Patient Population / Client Focus

Your primary focus will be on clients who are navigating the complexities of hospice care while dealing with mental health concerns related to terminal illnesses. You will engage with a diverse range of clients, providing support during one of the most challenging times in their lives, ensuring they feel heard, understood, and supported in their journey.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required for this position.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A minimum of two years of experience in a mental health or outpatient setting, particularly in roles involving hospice or palliative care, is preferred.
  • Exceptional interpersonal, communication, and assessment skills to effectively engage with clients and collaborate with multidisciplinary teams.
  • A strong understanding of and experience with evidence-based therapeutic modalities and a commitment to continuous professional development.
Preferred Experience
  • Previous experience working in hospice care or with terminally ill clients is highly desirable.
  • Knowledge of grief counseling techniques and the psychological aspects of end-of-life care is beneficial.
  • Familiarity with telehealth technology and best practices in remote therapy provision.
Compensation and Benefits
