Position Overview

We are seeking a dedicated and licensed Clinical Social Worker to join our team at Paradise Valley Behavioral Health. This full-time remote position allows you to work from the comfort of your home while making a meaningful impact in the lives of seniors through our hospice center. With a competitive annual salary range of USD 100,000 – 120,000, you will have the opportunity to provide vital support and advocacy for senior clients and their families.

Key Responsibilities
  • Conduct in-depth assessments of clients’ psychosocial needs, focusing on their emotional, mental, and social wellbeing.
  • Develop comprehensive care plans in collaboration with clients and their families, tailoring approaches to meet individual goals.
  • Provide counseling to seniors and their families, offering emotional support and guidance through life transitions, such as end-of-life issues and chronic illness management.
  • Advocate for clients’ rights by connecting them to essential community resources and services to meet their needs effectively.
  • Collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to ensure cohesive care delivery.
  • Regularly monitor client wellbeing through virtual check-ins, assessing safety in living conditions and addressing any critical concerns.
  • Facilitate communication among family members involved in the client's care, aiding in conflict resolution and collaborative caregiving.
  • Educate clients and families about local resources, support groups, and programs to encourage social engagement and improve overall quality of life.
  • Maintain thorough and accurate records of all client interactions, assessments, progress notes, and modifications to care plans in compliance with ethical standards and confidentiality laws.
  • Stay updated on best practices and current developments in social work, ensuring compliance with applicable regulations and standards.
Patient Population / Client Focus

This role specifically targets senior clients within a hospice care setting. You will work with individuals facing various psychosocial challenges related to aging, chronic health conditions, and transitions in care. Your skills will be crucial in enhancing their quality of life and providing support during difficult times.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution is required.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two years of professional experience in social work, ideally in settings focused on elder care or hospice support.
  • Comprehensive knowledge of social work principles, especially in the context of aging and community resources available for seniors.
