Key Responsibilities
  • Conduct virtual psychosocial assessments that comprehensively evaluate clients' emotional, social, and environmental strengths and needs.
  • Develop and implement evidence-based treatment plans tailored to individual client goals, particularly focusing on emotional support for patients and their families in a hospice setting.
  • Facilitate individual and group therapy sessions that foster coping strategies and emotional resilience, using techniques appropriate for patients experiencing grief and loss.
  • Work collaboratively with a multidisciplinary team, including physicians, nurses, and spiritual care providers, to coordinate integrated care and holistic support for clients.
  • Advocate for clients’ rights and facilitate access to community resources, ensuring that they receive necessary support and services throughout their treatment.
  • Meticulously document all client interactions, treatment progress, and outcomes in electronic health records, ensuring compliance with all applicable regulations and organizational standards.
  • Participate actively in team meetings, case conferences, and ongoing training sessions aimed at enhancing clinical skills and knowledge of best practices.
Patient Population / Client Focus

Your primary focus will be on patients facing significant health-related challenges, particularly those in hospice care who require psychological support during end-of-life transitions. You will work closely with families as well, providing guidance and support in navigating complex emotions such as grief, loss, and anticipatory mourning.

Qualifications
  • Master’s degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.
  • A minimum of two (2) years of post-licensure experience in providing clinical social work services, ideally in a healthcare or hospice setting.
  • Expertise in conducting diagnostic assessments and formulating comprehensive treatment plans.
  • Exceptional interpersonal and communication skills, with a demonstrated ability to engage effectively with a diverse client population.
  • A strong commitment to cultural competence, social justice, and ethical care practices.
Preferred Experience
  • Experience in hospice care or grief counseling is highly desirable.
  • Certification in evidence-based practices such as Dialectical Behavior Therapy (DBT) or Acceptance and Commitment Therapy (ACT).
  • Familiarity with electronic health record (EHR) systems, particularly for documentation and client management.
