• Conduct in-depth assessments of clients' psychosocial needs, utilizing established evaluation criteria to identify their emotional, social, and mental health requirements.
• Collaborate with clients and their families to construct personalized care plans that prioritize their specific goals and preferences, advocating for their best interests through the care process.
• Provide counseling and emotional support to clients, guiding them through significant life transitions and the challenges associated with aging.
• Serve as an advocate for clients' rights, ensuring access to essential community resources and services that can enhance their overall wellbeing.
• Work closely with interdisciplinary teams, including physicians, nurses, and caregivers, to coordinate effective care strategies and share insights on client welfare during regular team meetings.
• Conduct regular virtual visits to monitor clients' well-being, assess their environments, and address any emerging issues related to safety or quality of life.
• Facilitate family communication, addressing any conflicts or concerns to promote a better understanding of the care process among all involved parties.
• Educate clients and their families about available community supports, resources, and programs designed to foster engagement and socialization.
• Maintain thorough documentation of client interactions, assessments, and progress notes in compliance with ethical standards and state regulations.
• Keep abreast of current social work practices, policies, and the aging population's unique challenges to continually improve the quality of care provided.

In this role, you will primarily support elderly clients requiring end-of-life care and their families, advocating for their dignity and ensuring they receive compassionate and comprehensive support in a hospice environment.

• A Master’s degree in Social Work from an accredited program.
• Current and valid LCSW licensure in Texas.
• Minimum of two years of experience in social work, with a preference for backgrounds in hospice, palliative care, or community settings focused on geriatric populations.
• Strong knowledge of social work principles, especially in aging-related issues and available community resources.
• Exceptional counseling, assessment, and problem-solving abilities, demonstrating a genuine desire to help seniors navigate complex care needs.
• Empathetic communication style with excellent interpersonal skills, facilitating effective collaboration with various stakeholders.
• Proficiency in electronic health record (EHR) systems and other relevant software tools to document and manage client information effectively.

• Previous experience working in a hospice or healthcare environment.
• Familiarity with care coordination and interdisciplinary collaboration.
• Experience with geriatric counseling and advocacy.
