Key Responsibilities
  • Conduct thorough clinical assessments to evaluate the needs of patients and their families in hospice settings.
  • Develop and implement individualized treatment plans tailored to the specific challenges and goals set forth by each patient.
  • Utilize solution-focused therapeutic interventions such as grief counseling, crisis intervention, and resource linkage.
  • Maintain accurate and organized clinical documentation through our electronic health record system to assure timely care and compliance with regulations.

Patient Population / Client Focus

You will primarily serve patients who are in hospice care, often facing terminal illnesses. This role requires a strong understanding of the complexities involved in terminal care, including dealing with grief and loss, as well as navigating emotional and psychological distress for both patients and their families. Your efforts will directly contribute to improving the mental well-being of your clients, fostering a supportive environment during one of life’s most challenging times.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • A valid LCSW license in the state of Georgia is mandatory.
  • Proficient knowledge of solution-focused therapeutic practices and techniques.
  • Exceptional communication skills, both verbal and written, with a strong ability to empathize and connect with patients from diverse backgrounds.
Preferred Experience
