Work Environment

This position is primarily remote, allowing for flexible work arrangements while providing high-quality care to patients in a hospice center setting. Key Responsibilities

• Provide individualized psychotherapy to patients grappling with mental health conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive assessments to evaluate the mental, emotional, and social functioning of patients, utilizing standardized assessment tools and clinical judgment.
• Develop and implement evidence-based treatment plans that are culturally sensitive and tailored to meet the specific needs of each patient.
• Collaborate with a multidisciplinary healthcare team, including psychiatrists, nurses, and primary care providers, ensuring an integrated approach to patient care.
• Monitor patient progress through regular follow-up sessions, adjusting treatment plans based on therapeutic feedback and clinical outcomes.
• Educate patients and their families on mental health conditions, available treatment options, and community resources to empower informed decision-making.
• Maintain meticulous clinical documentation through electronic health records (EHR) to ensure compliance with health regulations and effective care coordination.
• Uphold ethical standards of practice by ensuring patient confidentiality, obtaining informed consent, and demonstrating professional integrity in all interactions.

Patient Population / Client Focus

This role mainly involves working with patients in a hospice facility, many of whom may be navigating significant life transitions. You will play an essential role in providing emotional support and mental health care at a sensitive juncture of their lives, addressing psychological distress with compassion and professionalism.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Minimum of 2 years of experience in providing therapy and mental health services, preferably in hospice or similar settings.
• Exceptional communication and interpersonal skills to foster strong therapeutic relationships with patients and collaborative partnerships with healthcare professionals.
• Solid clinical assessment skills, capable of developing and implementing effective treatment strategies.
• Proficiency in using electronic health record systems (EHR) for patient management and documentation.
• A comprehensive understanding of mental health conditions, treatment modalities, and the psychosocial challenges faced by patients.

Preferred Experience

• Experience in hospice care or palliative settings, with a focus on emotional and psychological support for terminally ill patients.

Compensation and Benefits
