• Conduct comprehensive initial assessments to evaluate patients' mental health needs and formulate appropriate intervention strategies.
• Design and implement individualized treatment plans that address the diverse psychological and social requirements of patients and their families.
• Provide both individual and group psychotherapy sessions, applying evidence-based therapies tailored to a range of mental health conditions.
• Perform crisis intervention for patients experiencing acute distress, utilizing de-escalation techniques and creating safety plans.
• Maintain meticulous documentation of patient progress and treatment outcomes through our electronic medical records system, ensuring compliance with industry standards and confidentiality protocols.
• Collaborate closely with interdisciplinary healthcare teams, including physicians, nurses, and other specialists, to synchronize patient care efforts and support holistic treatment plans.
• Participate in comprehensive staff meetings and ongoing clinical supervision workshops to refine your practice and share insights from clinical experiences.
• Stay informed about the latest research and developments in mental health, integrating innovative approaches into your practice to enhance patient care.

You will primarily work with individuals facing mental health challenges related to terminal illnesses, end-of-life issues, and grief. Your empathetic approach will support patients and their loved ones in coping with emotional strain, navigating complex family dynamics, and fostering resilience in the face of challenging circumstances. Understanding cultural sensitivities and the importance of individualized care will be paramount in your practice.

• A Master’s degree in Social Work (MSW) from an accredited institution, demonstrating a strong foundation in therapeutic principles and practices.
• A current and valid Licensed Clinical Social Worker (LCSW) license in the state of California, underscoring your qualifications to offer clinical services.
• Demonstrated expertise in providing therapy to a diverse client base, especially those experiencing mental health conditions associated with chronic or terminal illnesses.
• Exceptional communication, active listening, and interpersonal skills to effectively engage with patients and collaborate with multidisciplinary teams.
• Proficient in utilizing electronic medical records and related health informatics tools for comprehensive patient care management.

• Prior experience in a hospice or palliative care setting, with familiarity in addressing the psychological aspects of end-of-life care.

• Knowledge of community resources and referral pathways to assist patients in accessing additional support systems and services.

The salary range for this role is competitive, between USD 80,000 and 100,000 annually, commensurate with experience and qualifications.
