• Conduct thorough assessments of patients to identify mental health needs and develop personalized, goal-oriented treatment plans.

• Implement evidence-based psychotherapeutic interventions tailored to the hospice environment, ensuring patient comfort and dignity.

• Maintain accurate, confidential clinical records in compliance with ethical and legal standards, utilizing our secure electronic health record system.

• Collaborate regularly with a multidisciplinary treatment team, including physicians, nurses, and spiritual care counselors, to provide coordinated care.

• Educate patients and families about mental health resources, coping strategies, and available hospice services to promote informed decision-making.

• Stay abreast of the latest mental health research and clinical developments, participating in ongoing training and professional development opportunities.

In this role, you will primarily serve patients and their families in hospice care, addressing the emotional and psychological challenges associated with terminal illness and bereavement. Your expertise will directly contribute to enhancing patients' quality of life, facilitating discussions around end-of-life wishes, and providing support during a time when emotional resilience is crucial.

• Master’s degree in Social Work from an accredited institution is required.

• A current, valid LCSW license in the state of Virginia.

• Minimum of 2 years clinical experience in a mental health or social services setting, ideally with experience in hospice or palliative care.

• Proficiency in solution-focused and trauma-informed therapeutic modalities.

• Exceptional communication and interpersonal skills with a strong aptitude for empathy and compassion.

• Experience working with diverse populations and understanding cultural sensitivity in mental health practices.

• Familiarity with grief counseling and loss process interventions.

• Ability to thrive in a collaborative team environment, contributing valuable insights and a compassionate approach to care.
