• Conduct comprehensive clinical assessments to evaluate clients’ mental and emotional well-being, and develop individualized treatment plans that employ evidence-based, solution-focused interventions.
• Implement therapeutic strategies tailored to clients' unique needs, ensuring adherence to the highest professional and ethical standards.
• Maintain meticulous clinical documentation, ensuring timely and accurate record-keeping that complies with regulations and organizational policies.
• Collaborate with a multidisciplinary team including physicians, nurses, and other mental health professionals to provide holistic care to patients and their families.
• Participate in regular case consultations and peer review meetings to enhance treatment quality and patient outcomes.
• Provide education and resources to patients and families about mental health and the coping mechanisms relevant to their experiences.
• Stay informed about the latest research and advancements in mental health practices to continuously improve care delivery.

This role specifically focuses on patients facing terminal illness, who often experience complex emotional and psychological challenges. The LCSW will need to employ strong empathy and resilience in guiding clients and their families through grief, loss, and anticipatory sorrow. Understanding the profound impact of mental health on end-of-life care, you will play a pivotal role in enhancing patient dignity and quality of life.

• Must possess a Master’s degree in Social Work from an accredited institution.
• Valid LCSW license in the state of Colorado.
• At least three years of clinical experience with a focus on psychotherapy, with a preference for experience in hospice or palliative care settings.
• Demonstrated proficiency in solution-focused therapeutic approaches.
• Exemplary communication skills, both verbal and written, with a strong ability to engage with diverse populations.
• A collaborative mindset, thriving in a team-oriented environment.

• Experience working within a hospice or similar healthcare environment, with familiarity in navigating complex end-of-life conversations.
• An understanding of and ability to implement trauma-informed care practices.
• History of leveraging telehealth platforms effectively to deliver mental health services.
