This position is entirely remote, allowing for flexibility in your work schedule. As part of a community health clinic, you will work in collaboration with a multidisciplinary team of professionals dedicated to patient-centered care. • You will report directly to the Social Work Manager and be responsible for evaluating and diagnosing a wide range of psychosocial issues faced by patients.
• Utilizing individual clinical social work skills, your role involves assisting patients and their families in overcoming personal and environmental challenges related to mental and physical health concerns.
• Conduct comprehensive bio-psychosocial assessments to identify barriers to treatment and facilitate patient discharge, ultimately improving health outcomes.
• Work closely with the medical and support staff to develop integrated care plans that enhance treatment compliance and coordination of care.
• Create and implement individualized treatment plans that utilize evidence-based practices, with a focus on achieving optimal patient health status.
• Document all social work activities consistent with departmental standards to ensure high-quality patient care and compliance with regulatory guidelines.
• Educate patients and their families on medical and social work recommendations to promote understanding and adherence to treatment plans.
• Advocate for patients by assessing their needs and connecting them with community resources to foster self-determination and social support.
• Maintain up-to-date knowledge of local community resources relevant to patient care needs.
• Engage in ongoing professional development to enhance your clinical skills and knowledge through relevant training and educational activities.
• Ensure adherence to departmental policies and procedures to maintain a safe and compliant work environment.

As an LCSW at Refreshed Mind Clinic, you will work with a diverse group of patients from various backgrounds and age groups. Your focus will be on providing care that considers the unique psychosocial factors impacting their mental health. This includes addressing challenges associated with aging, cultural diversity, and varying levels of health literacy, ensuring that each patient receives tailor-made interventions suited to their specific needs.

• A Master's Degree in Social Work from an accredited graduate program, along with a current LCSW license from the State Board of Social Work Licensure.
• A minimum of one year of clinical experience in a health or mental health setting, preferred work in similar populations relevant to our services.
• A demonstrated understanding of the aging process, human development stages, and cultural considerations in treatment planning.
• Strong analytical skills to evaluate patient progress, develop treatment plans, and adjust interventions based on outcomes.

• Experience working within a community health clinic or outpatient setting.
• Familiarity with telehealth platforms and digital health tools to facilitate remote patient care.
• Proficiency in multiple languages is a plus, aiding in communication with non-English speaking patients.
