Key Responsibilities
  • Coordinate closely with Reentry Social Workers to craft comprehensive discharge plans that meet the unique needs of each patient.
  • Prepare and submit statistical reports in a timely manner to track patient outcomes and support clinic objectives.
  • Collaborate with the Department of Correction to facilitate patient access to services, housing, and mental health treatment plans.
  • Develop and lead therapeutic group sessions, targeted at mental health observation units and other assigned locations, utilizing a variety of group therapy methodologies.
  • Ensure meticulous documentation for all patient interactions and treatment plans, maintaining compliance with organizational standards and regulations.
  • Engage in weekly clinical supervision sessions with a Clinical Supervisor, promoting professional growth and satisfaction.
  • Take part in mental health rounds and case conferences to discuss treatment options for current and prospective patients.
  • Conduct mental health evaluations, crisis interventions, and follow-up assessments to ensure ongoing quality care.
  • Assess and determine the appropriate level of mental health care for patients, coordinating with various institutional programs as needed.
  • May oversee social work interns or students, providing mentorship and guidance in clinical practices.
