Key Responsibilities

  • Maintain up-to-date crisis alerts to ensure seamless coordination with county crisis services, assisting in the rapid delivery of necessary support when required.
  • Oversee the transition of clients between hospital and outpatient settings, collaborating with hospital personnel to facilitate effective discharge and follow-up care.
  • Document all interactions with clients and other stakeholders accurately in our electronic health records system, ensuring continuity of care.

Patient Population / Client Focus
