Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients’ emotional, social, and financial needs, collaborating with medical teams to formulate holistic care plans.
  • Provide individual and group counseling sessions to address issues such as grief, adjustment disorders, and coping strategies, empowering patients to navigate their health challenges effectively.
  • Coordinate discharge planning and collaborate with community organizations to ensure continuity of care post-discharge.
  • Offer emotional support and educational resources to patients and their families regarding treatment options, available services, and coping mechanisms.
  • Develop and implement effective discharge plans tailored to individual patient requirements and connect them with appropriate community resources.
  • Provide crisis intervention and counseling during critical situations, ensuring patients and families receive prompt and compassionate support.
  • Maintain accurate and timely documentation of all interactions and assessments in compliance with hospital policies and legal requirements.
  • Participate actively in interdisciplinary team meetings and contribute to hospital committees to enhance practices related to social work.
Patient Population / Client Focus

In this position, you will primarily work with patients facing terminal illnesses, providing them and their families with critical emotional support and resources to improve their quality of life. You will engage with a diverse client base, understanding their unique circumstances and helping them to cope with potentially challenging life transitions.

Qualifications
  • A Master’s degree in Social Work from an accredited program.
  • An active state license as a Licensed Clinical Social Worker (LCSW).
