Key Responsibilities

  • Participate in hospital committees to provide social work insights and improve clinical practices.
  • Develop and execute comprehensive discharge plans that connect patients with proper community resources.
  • Facilitate crisis intervention sessions, providing immediate support to patients and families in distress.
  • Maintain detailed and timely documentation in accordance with legal requirements and clinic protocols.
Patient Population / Client Focus

You will be working with diverse patient groups facing a variety of mental health challenges. This includes individuals coping with chronic illnesses, mental health disorders, and those undergoing significant life changes. Your role is essential in ensuring that psychological and emotional needs are met as part of their overall healthcare journey.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong knowledge of mental health disorders and crisis intervention techniques.
Preferred Experience
  • A minimum of two years working in a healthcare or clinical setting is preferred.
  • Experience conducting psychosocial assessments and providing therapy in a clinical environment.
  • Familiarity with EMR systems and documentation practices in accordance with healthcare regulations.
