• Conduct thorough assessments and create tailored treatment plans to address the needs of clients, including pre-crisis and crisis planning.
• Document clinical interactions diligently, ensuring compliance with regulatory standards and timely completion of required paperwork.
• Utilize evidence-based screening tools to evaluate client needs and facilitate effective treatment strategies.
• Work collaboratively with interdisciplinary teams to design and implement positive behavior support plans.
• Engage actively in licensing and accreditation protocols, incorporating feedback into clinical practice.
• Serve as a liaison to relevant service systems, state agencies, and community partners, ensuring coordinated care for clients.
• Maintain effective communication with various stakeholders to uphold contract standards and further the mission of Grace Hospital within the community.
• Establish and nurture relationships with local and state organizations to create strategic partnerships and identify funding opportunities.
• Engage in ongoing professional development opportunities, including attending conferences and representing the agency at statewide meetings and task forces.
• Embody the values of the organization by promoting agency mission, philosophies, and principles in all interactions.
• Perform additional duties as assigned by the Clinic Director.

As an LMHC at Grace Hospital, you will work with a diverse client population that includes individuals facing a variety of emotional health challenges, from anxiety and depression to substance use recovery. Your role will focus on developing individual care plans that emphasize holistic and client-centered approaches to foster recovery and wellness pathways.

• A Master's Degree in a behavioral health field is required.
• Possession of an independent LMHC License from the state of Connecticut is mandatory.
• Proven experience in maintaining clinical records and preparing comprehensive reports.
• Strong ability to collaborate within an interdisciplinary team, ensuring integrated care delivery.
• Effective communication skills, with a talent for articulating ideas and clinical concepts clearly.
• Sound judgment abilities, allowing for independent decision-making based on professional expertise.

• Previous experience in a community health setting or similar environment focused on mental health services.
• Familiarity with evidence-based practices and treatment modalities relevant to mental health.
• Experience with crisis intervention and the development of crisis plans.
