• Conduct thorough assessments and develop individualized treatment plans, including crisis plans and medication treatment plans, ensuring they align with regulatory standards.
• Document all interactions and clinical findings promptly and accurately, utilizing electronic health record systems to maintain compliance.
• Employ evidence-based screening tools as appropriate to support the clinical decision-making process.
• Collaborate with multidisciplinary teams to craft effective positive behavior support plans tailored to client needs.
• Actively participate in licensing and accreditation processes, ensuring that our practices meet necessary guidelines.
• Serve as a liaison between clients and various service systems, including state agencies and community partners, to facilitate seamless care delivery.
• Foster open communication with stakeholders to uphold contract standards and to further promote our organization’s mission in the community.
• Establish relationships with local, state, federal, and private organizations to enhance program visibility and identify potential funding sources.
• Engage in professional development opportunities, including attending conferences and participating in task forces relevant to mental health.
• Actively integrate the core mission, values, and philosophies of Grace Hospital into your daily practice.
• Perform additional duties as assigned by the Clinic Director to support operational excellence.

This role will serve a diverse patient population requiring varying levels of mental health support, including individuals struggling with addiction, developmental disabilities, and adjustment disorders. You will develop therapeutic relationships that empower clients to navigate their recovery journeys while promoting community engagement and personal independence.

• A Master’s Degree in a behavioral health field is required for this position.
• Possession of a valid and independent LMHC license from the state board.
• Strong capability to maintain clinical records and produce related documentation efficiently.
• Ability to thrive in an interdisciplinary team, showcasing strong collaborative skills and effective communication.
• Demonstrated judgment and decision-making skills based on clinical expertise and ethical standards.

• Experience in telehealth or remote service delivery, adapting therapeutic practices to an online environment.
• Previous involvement with crisis intervention planning and implementation.
• Knowledge of evidence-based methodologies that can be applied in treatment planning.
