You will collaborate with interdisciplinary teams, including physicians, nurses, and spiritual care counselors, to ensure comprehensive care that fosters emotional support and guidance throughout the treatment journey.

• Conduct thorough evaluations of patients' psychological and emotional needs, utilizing interviews and analytic observations to gather relevant data.
• Facilitate therapy sessions targeting mental health conditions such as anxiety, depression, end-of-life concerns, addiction, and chronic pain, tailoring interventions to individual patient needs.
• Work collaboratively with healthcare professionals to develop empathetic care plans that integrate psychological and medical approaches.
• Maintain accurate and complete medical records in compliance with healthcare regulations and standards.
• Provide education and consultation to families and community stakeholders, promoting understanding and awareness of the social determinants impacting patient care.
• Support complex discharge planning by coordinating community resources and services to ensure smooth transitions.

This role focuses on patients facing life-limiting conditions who require emotional and psychological support as they navigate their unique healthcare journeys. You will work closely with patients, their families, and significant others to provide compassionate care during challenging times, fostering a supportive atmosphere that prioritizes patient dignity and emotional well-being.

• Master’s degree in Social Work from an accredited institution.
• Active state license as a Licensed Clinical Social Worker (LCSW) is required.
• A minimum of 2 years of related experience in a clinical setting, preferably with hospice or palliative care populations.
• Demonstrated ability to maintain professional boundaries and uphold patient confidentiality at all times.
• Strong verbal and written communication skills, with an ability to connect empathically with patients and families.

• Experience in hospice care or working with terminally ill patients is highly desirable.
• Familiarity with interdisciplinary team dynamics and collaborative care approaches.
• Proficiency in electronic health record (EHR) systems to manage patient documentation effectively.
