As a Licensed Clinical Social Worker at Optimum Care Center, your daily responsibilities will include:

• Conducting thorough clinical assessments to understand the specific mental, emotional, and social needs of patients and their families.
• Developing patient-centered treatment plans incorporating best practices and evidence-based interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).
• Providing high-quality psychotherapy and counseling services, ensuring all interactions contribute positively to the patient’s mental health journey.
• Maintaining accurate clinical documentation in compliance with HIPAA regulations, enhancing both patient care and organizational integrity.
• Collaborating with a multidisciplinary team, including medical doctors, nurses, and therapists, to coordinate comprehensive care for patients.
• Participating in case consultations and team meetings to continuously improve treatment strategies and approaches.
• Offering education and support to patients and their families about mental health issues, coping strategies, and available resources.
• Staying current with the latest research and trends in social work and mental health practices for ongoing professional development.

You will primarily work with patients who are dealing with chronic diseases and are in hospice care. Understanding their unique emotional needs is crucial, as they may experience a range of feelings including anxiety, despair, and uncertainty. Your role will help facilitate open discussions about their emotional health and assist families in coping with these challenges during sensitive times.

• A Master's degree in Social Work from an accredited institution.
• A valid state license to practice as a Licensed Clinical Social Worker (LCSW).
• A minimum of 3 years of direct clinical experience, particularly in providing psychotherapy services.
• Proficiency in solution-based treatment methods for diverse clientele.
• Strong interpersonal and communication skills, with an ability to collaborate effectively within a team.
• Demonstrated problem-solving skills and critical thinking abilities crucial for tailoring treatment plans to individual patient needs.
• A commitment to adhering to ethical standards as set forth by the National Association of Social Workers (NASW).

• Experience in hospice or palliative care settings is highly preferred.
