Your work will primarily take place online, providing services to patients and families via telehealth platforms. • Conduct thorough mental health assessments to evaluate the psychological needs of hospice patients and their families, taking into account their experiences, fears, and coping mechanisms during end-of-life care.
• Develop, implement, and regularly revise individualized treatment plans tailored to enhance emotional well-being and ensure supportive care throughout the hospice journey.
• Utilize evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT) and mindfulness techniques, to provide tailored interventions that support patients' emotional needs.
• Facilitate individual and group therapy sessions aimed at fostering emotional resilience and helping patients and families navigate grief and loss.
• Collaborate closely with an array of healthcare professionals, including physicians, nurses, and chaplains, to ensure an integrated approach to patient care and support comprehensive treatment strategies.
• Provide crisis intervention strategies to address urgent mental health needs and facilitate appropriate referrals to higher levels of care when necessary.
• Engage with families through psychoeducation, empowering them to better understand the mental health aspects of the hospice experience and available resources.
• Ensure precise documentation and maintenance of treatment records, assessments, and follow-up notes in accordance with professional standards and legal requirements.
• Participate in team meetings and training sessions to foster interprofessional collaboration and enhance care delivery practices.

This role focuses on hospice patients of all ages facing life-limiting illnesses, along with their families. You will play a crucial role in providing emotional support to individuals navigating complex emotional landscapes, grief, and transitional phases, ensuring that their needs are prioritized with compassion and attentiveness.

• Master’s degree in Social Work from an accredited program.
• Valid LICSW licensure in Vermont.
• A minimum of two years post-licensure experience in a clinical or healthcare setting, with a strong preference for experience in hospice, palliative care, or related environments.
• Demonstrated knowledge of mental health disorders, crisis intervention strategies, and trauma-informed care practices.
• Exceptional skills in conducting mental health evaluations, diagnosing conditions, and developing comprehensive treatment plans.
• Experience in providing therapy to individuals and groups, applying a variety of therapeutic modalities.
• Strong conflict resolution, communication, and interpersonal skills, with the ability to form trusting relationships with diverse populations.
• Familiarity with electronic health records (EHR) and digital platforms for telehealth services.

• Previous experience in a hospice or end-of-life care environment.
• Expertise in grief counseling and palliative care methodologies.
• Knowledge of community resources and additional support services that can aid patients and families.

The position offers a competitive salary ranging from USD 80,000 to 100,000 per year, commensurate with experience and qualifications.
