• Conduct detailed clinical assessments to identify patients' psychosocial, emotional, and practical needs.
• Develop personalized treatment plans employing evidence-based practices and solution-focused methodologies tailored to each patient.
• Implement and evaluate therapeutic interventions, ensuring continual alignment with clinical goals and patient progress.
• Maintain accurate and timely documentation of clinical records in accordance with regulatory requirements, ensuring compliance and quality care.
• Collaborate effectively within a multidisciplinary team, including medical staff, nurses, and other social workers, to coordinate care plans.
• Participate in case consultations and peer review to uphold the highest standards of clinical excellence.
• Provide education and support to patients and their families, empowering them with knowledge regarding mental health resources and coping strategies.
• Engage in continuous professional development by remaining informed about the latest research, trends, and best practices in social work and hospice care.

You will work closely with a diverse patient population that includes individuals diagnosed with terminal illnesses, as well as their family members facing the emotional impact of such conditions. Your expertise will be crucial in addressing the psychological and social aspects of care, ensuring that patients and families navigate complexities with support and compassion.

• Master’s degree in Social Work from an accredited institution.
• A valid and active LCSW license in the state of California.
• A minimum of 2 years of clinical experience in a related field, particularly in hospice, palliative care, or mental health settings.
• Demonstrated proficiency in solution-focused therapy and evidence-based clinical interventions.
• Exceptional communication and interpersonal skills, capable of building rapport and trust with patients and their families.
• A strong sense of teamwork and the ability to thrive within a collaborative healthcare environment.

• Experience working with patients in a hospice or palliative care environment.
• Familiarity with electronic health record (EHR) systems to maintain clinical documentation and patient data.
• Background in facilitating support groups or educational workshops for patients and families dealing with grief and loss.
