You will collaborate closely with a multidisciplinary team, including healthcare providers, therapists, and caregivers, to deliver comprehensive care and services. • Provide remote clinical social work services, assessing the psychosocial needs of clients and their families.
• Conduct detailed assessments and develop individualized treatment plans aimed at addressing emotional, social, and practical needs.
• Coordinate services and resources to ensure clients' rights and access to essential support, enhancing their quality of life during hospice care.
• Advocate for clients, working towards the removal of barriers and ensuring equitable access to community resources and palliative care services.
• Collaborate with our interdisciplinary team to ensure the delivery of integrated and holistic care, facilitating communication among all stakeholders involved in client care.

The primary focus of this role is to support patients in hospice care, addressing the unique emotional and psychological challenges they face during this sensitive period. You will engage with clients dealing with terminal illnesses and their families, providing guidance, grief counseling, and emotional support tailored to their specific circumstances.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Licensure as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of 3 years of clinical social work experience, preferably in hospice, palliative care, or similar settings.
• Exceptional communication skills, with the ability to interact compassionately and effectively with clients and families.
• Proficiency in remote work technologies and a willingness to embrace innovative tools and systems to enhance client interaction.

• Experience working within hospice or palliative care settings.
• Knowledge of community resources and support services pertinent to end-of-life care.
• Familiarity with grief counseling methodologies and trauma-informed care practices.
