Key Responsibilities

• Conduct thorough assessments of patients to determine their social, emotional, and resource needs.
• Develop and implement individualized treatment plans based on assessment outcomes, treatment goals, and client collaboration.
• Provide counseling and support services, utilizing evidence-based practices tailored to each client's unique situation.
• Advocate for patients' rights and coordinate care with healthcare professionals, families, and community resources to optimize patient access to essential services.

• Educate patients and families about available hospice services and support, ensuring they understand their rights and the resources at their disposal.

Patient Population / Client Focus

As a Remote Licensed Clinical Social Worker at Radiant Life Psychological Services, your primary focus will be on providing compassionate care to patients in a hospice center. This includes individuals and families experiencing terminal illnesses, navigating grief and loss, or requiring support with complex emotional and practical issues related to end-of-life care. Your work will contribute significantly to enhancing patients' quality of life, ensuring their dignity is upheld during this critical time.

Qualifications

• A Master's degree in Social Work (MSW) from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) certification in California.
• A minimum of three years of professional experience in social work, particularly in healthcare or hospice settings.
• Strong communication skills, empathic listening abilities, and a high level of cultural competence.
• Proficiency with telehealth and remote work technologies to facilitate effective service delivery.

Preferred Experience

• Experience in hospice, palliative care, or end-of-life counseling preferred.

Compensation and Benefits
