Key Responsibilities

• Conduct comprehensive psychosocial assessments to understand clients’ mental health concerns, strengths, and resources.
• Deliver individual, family, and group therapy sessions employing evidence-based treatment modalities to promote wellness.
• Utilize therapeutic approaches such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care to address clients' unique challenges.
• Develop and implement individualized treatment plans that include specific goals and interventions tailored to each client's needs.
• Collaborate closely with multidisciplinary teams, including psychiatrists and psychologists, to ensure a cohesive care approach.
• Engage in crisis intervention, conduct risk assessments, and develop robust safety plans when necessary.
• Maintain accurate, timely, and compliant documentation in accordance with ethical and legal standards.
• Facilitate psychoeducational classes, support groups, and group therapy sessions that enhance community and client engagement.

Patient Population / Client Focus

In this role, you will work with a diverse client base, including individuals and families facing various mental health challenges, particularly in a hospice context. Your expertise will help to support clients through grief, anxiety, and other emotional struggles associated with end-of-life care. Qualifications

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
