Key Responsibilities

As an LCPC, you will be responsible for a range of crucial tasks that directly impact the lives of our patients:

• Conduct comprehensive assessments to determine the mental health needs of each patient, ensuring personalized care.
• Facilitate clear communication among supervisors, direct care staff, families, and other stakeholders regarding patient progress.
• Develop and maintain a tentative schedule for appointments, home visits, and observations to monitor patient progress effectively.
• Coordinate in-home visits, including chart reviews and staff interviews, to deliver thorough data analysis and observation.
• Oversee the implementation of individual treatment plans, ensuring they are executed effectively and appropriately.
• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on treatment strategies.
• Document bi-monthly and quarterly progress notes in a timely manner to track patient outcomes and improvements.
• Organize training sessions to equip staff with competencies for implementing program plans and documentation procedures.
• Offer follow-up support through interviews, home visits, and observations to reinforce the care process.
• Maintain accurate patient records, guaranteeing that information is accessible to authorized individuals, thereby supporting continuity of care.
• Act as a liaison within the public and mental health communities to promote development initiatives.
• Contribute to the creation of training materials aimed at enhancing program efficacy for our clients.

Patient Population / Client Focus

The ideal candidate will work with a diverse range of clients, including both adults and youth, facing various mental health challenges. Qualifications

To succeed in this role, candidates should possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Current, active, and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
• A minimum of two years of relevant experience in counseling, ideally within a clinical context.
• Preferred specialization in areas such as family therapy, couples therapy, substance use disorders, or trauma-informed care.
• A strong foundation in legal and ethical standards governing mental health practices.
• Exceptional interpersonal and communication skills to build rapport with a diverse clientele.
• A demonstrated ability to collaborate effectively within a multidisciplinary healthcare team.
• Familiarity with electronic medical record (EMR) systems and related technologies.

Preferred Experience

While not mandatory, additional expertise in specific therapeutic modalities or working with particular populations will be advantageous. Candidates who hold certifications in trauma-informed care or substance misuse counseling will stand out in the selection process.

Compensation and Benefits

We offer a competitive annual salary ranging from $90,000 to $110,000, commensurate with experience and case load capacity. Comprehensive benefits include:
