• Conduct comprehensive psychosocial assessments to identify the individual needs of patients and their families within the hospice care framework.
• Provide consultations to interdisciplinary team members regarding the psychosocial aspects of patient care, focusing on enhancing compliance with treatment protocols and improving health outcomes.
• Engage actively in professional peer review case conferences, contributing insights during discussions that inform best practices and treatment modifications.
• Utilize a thorough understanding of medical diagnoses and treatment procedures, including the impact of chronic and terminal illnesses, to guide therapeutic interventions.
• Collaborate with internal departments and local community organizations to facilitate resources and support systems that benefit the patients and their families.
• Participate in ongoing supervision and peer support to evaluate and enhance professional clinical competencies while seeking opportunities for personal and professional growth.

This role focuses on serving patients at the end of life, who face not only physical challenges but also emotional, social, and spiritual concerns. Our clinical social workers support patients and families by addressing grief, anxiety, and existential distress while fostering a sense of peace and dignity during the hospice experience.

• A Master's degree in Social Work from an accredited program is required, ensuring a strong foundation in clinical practice.
• Current licensure as a Clinical Social Worker (LCSW-R) in the state of New York is essential for this role.
• A minimum of 2 years of post-graduate clinical experience in a relevant setting, especially within outpatient or hospice environments, is necessary to ensure qualified, compassionate care.

• Prior experience in a leadership role, demonstrating effective communication and collaboration skills within a multidisciplinary team.
• Familiarity with hospice or palliative care models and an understanding of the challenges faced by individuals nearing the end of life will enhance your ability to connect with patients and families.

We offer a competitive salary range of $90,000 to $110,000 per year, commensurate with experience, along with a robust benefits package that includes health insurance, paid time off, and retirement savings options.
