The Care Coordinator will be the primary care manager for a panel of serious mental illness/SMI members with varying risk and may be assigned other health plan populations as needed.

This position is responsible for completing Health Risk Assessments, Individualized Service Plans and linking members to needed services to ensure that the members are receiving the healthcare they need. Members worked within this position are those with behavioral health needs. This position would involve working with physicians, behavioral health providers, and other community stakeholders as well. Care Managers working with this population will be required to work in the field to complete face-to-face visits for 25-50% of assigned members.

Care coordination activities will focus on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. Care Coordinator will be responsible for collaborating and coordinating care with community partners such as the CSB.


This is a Field Based Position with a Home-Based Office.

Required Qualifications:

  • Bachelor's degree in health or human services Field OR LMHP OR RN/LPN OR QMHP OR LMSW OR LBSW OR MSW OR BSW
  • 1 years of care coordination or behavioral health experience and/or work in a healthcare environment
  • 1 years of experience with MS Office, including Word, Excel, and Outlook
  • Driver's license and reliable transportation and the ability to travel within assigned territory to meet with members and providers if required


Preferred Qualifications:

  • CCM certification
  • QMHP
  • Experience working with Medicaid/Medicare population
  • Long term care/geriatric experience
  • Experience working in team-based care
  • Background in Managed Care

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $23.89 to $42.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
