In your role as a Licensed Professional Counselor, you will be responsible for:

• Conducting comprehensive evaluations to understand the needs and goals of patients.
• Facilitating effective communication among supervisors, direct care staff, and family members to monitor and support individual development.
• Preparing detailed care plans outlining appointments, home visits, and observation sessions to track and assess progress over time.
• Organizing in-home observation visits, which may include reviewing charts, interviewing staff, and collecting relevant data for thorough analysis.
• Ensuring the implementation of treatment strategies, adapting as necessary to meet patient needs.
• Engaging in interdisciplinary team meetings to coordinate care and share insights.
• Documenting monthly and quarterly progress notes to track treatment efficacy and patient evolution.
• Arranging training and development sessions to ensure staff proficiency in implementation and documentation practices.
• Conducting follow-up visits and assessments to evaluate ongoing patient needs and ensure continuity of care.
• Maintaining accurate and current records, ensuring crucial information is accessible for program assessment and continuity.
• Serving as a liaison between the clinic and community partners to foster collaborative relationships that enhance service delivery.
• Assisting in the development of training materials that empower team members and improve program effectiveness.

Your work will contribute to a diverse patient base, serving both adults and youth. You will focus on varying mental health challenges, including but not limited to trauma, family dynamics, and substance abuse issues. Your empathetic approach and specialized training will help guide clients through their journeys toward recovery and personal growth.

• A minimum of a Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential.
• Current and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey is required.
• A minimum of two years of counseling experience, particularly in a clinical context, is preferred.
• Specialization in family therapy, couples therapy, trauma, or substance misuse is highly sought after.
• Comprehensive understanding of legal and ethical standards in mental health practice.
• Exceptional interpersonal and communication skills, with the ability to build rapport with a diverse clientele.
• Proven ability to collaborate effectively with a multidisciplinary healthcare team.
• Familiarity with electronic medical records (EMR) software and related technologies is beneficial.
