This role will be conducted remotely, with a focus on outreach and support services for seniors. Key Responsibilities
  • Conduct thorough psychosocial assessments of seniors, evaluating emotional, social, and mental health factors that impact their well-being.
  • Collaborate with clients and their families to craft personalized care plans that align with individual goals, preferences, and life situations.
  • Provide counseling and emotional support to clients navigating challenges, including transitioning to senior living facilities or managing health issues.
  • Advocate for clients’ rights while facilitating their access to valuable community resources and support services.
  • Work closely with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate services and enhance care delivery.
  • Perform home or community visits to monitor clients' living conditions, assess their safety, and adjust care plans as necessary.
  • Facilitate family meetings to promote communication and resolve conflicts involving clients’ care.

  • Document all client interactions, assessments, and treatment plans accurately within our electronic health record (EHR) systems to ensure compliance and continuity of care.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution is required.
